Incidence of Metallo Beta Lactamase Producing Gram Negative Non Fermenters in a Tertiary Care Hospital of Navi Mumbai

ABSTRACT:

Metallo beta lactamase producing gram negative non fermenters (GNNF’S) have been reported to be important cause of nosocomial infections. Several studies in India have shown the incidence of MBL production among GNNF’S to be 10 – 30 %. Emergence of such multi drug resistant strains is posing a therapeutic challenge with limited treatment options. Hence prompt detection of metallo beta lactamase (MBL) producing strains is necessary to prevent their dissemination .In our study a total of 165 GNNF’S were isolated from various clinical samples such as urine, pus, sputum, endotracheal tube secretions. Imipenem & Meropenem resistant isolates were selected for MBL production by Imipenem – EDTA combined disc method. Among these 165 GNNFs isolated, 110 were P.aeruginosa, 51 were Acinetobacter baumannii & 4 were P.fluorescens. Among these, 22(20%) P.aeruginosa, 19(37.25%) Acinetobacter baumannii, 01(25%) P. fluorescens were Imipenem resistant. 13 strains of P.aeruginosa, 11 of Acinetobacter baumannii & 01 of P.fluorescens were found to be MBL producers. MBL producing strains were isolated maximally from urine samples (10), followed by pus (8) & endotracheal tube secretions (7). Out of the total 165 GNNF’S isolated, 25 isolates were MBL producers. Thus in our study the incidence of MBL production among GNNF’S was 15.15%.
